Union Associated Physicians Clinic (UAP Clinic) and Indiana State University are pleased to announce a new partnership. Under the new arrangement, UAP Clinic is being contracted by ISU to be the provider of care at the health center.
UAP Clinic and ISU entered into this partnership as a cost-effective way to continue providing high quality healthcare to ISU students. UAP Clinic will leverage its core business as a healthcare provider to assist ISU to enhance services available via the health center.
UAP Clinic is a primary and specialty care medical practice with nine clinics throughout Terre Haute. The UAP family of clinics, comprised of physicians, nurse practitioners, physician's assistants and more than 500 staff members, offers services 21 medical specialties.
ISU Clinical staff at the health center will have access to a state-of-the-art electronic health record. In addition, UAP Clinic will be installing a patient portal that will allow students to view key parts of their medical records, request appointments online and communicate with the health center via secure e-mails.
The clinical employees at the health center are UAP Clinic employees. The UAP Clinic – ISU Health Center is staffed by a nurse practitioner, laboratory and x-ray techs, nurse and support staff. In the fall, physicians will supplement the clinical staff at the health center.
Services available at the health center include immunizations, men's and women's health services, educational materials, STD screenings/counseling and limited amounts of prepackaged medications.
The UAP Clinic – ISU Health Center remains located at its current location at 567 N. Fifth St. on the ISU campus. This is across the street from the tennis courts on 5th, just north of the student union building and right next to Lincoln Quad residence hall.
All undergraduate and graduate students taking one or more credit hours at ISU are eligible to receive services at the health center. Students must present a valid ISU identification card in order to receive services.
Students seeking services from the UAP Clinic – ISU Health Center should bring their insurance card with them to their appointment. UAP Clinic – ISU Health Center participates in many insurance plans. However, payment of plan co-payments is expected at the time of service. Any balance left unpaid by the insurance company will be sent to the person listed as guarantor.
UAP Clinic - ISU Health Center accepts the Associated Insurance Plan, private insurances, cash, check or credit cards.
Students with insurance coverage need to bring their card with them when they visit the health center. Plan co-payments are due at the time of service. Any balance due after insurance will be billed to the guarantor.
Students without insurance coverage are required to pay the office visit
fee at the time of their visit. The fees are as follows:
New Patient - $20
Established Patient - $15
Preventative Visit New and Established Patient - $25
It is important to note there may be charges for radiology, laboratory, physical therapy, allergy and immunization, urgent care services, special procedures, medications and supplies at the UAP Clinic ISU Health Center. In this case, a payment plan for any remaining balance can be established.

In case of an emergency on campus, call the University Police dispatch operation at (812) 237-5555 to report the incident. University Police will respond, contact an ambulance and direct it to the site of the emergency in order to provide transportation as needed.
